Registering with the surgery
To register with the surgery:
- complete the NHS: register with a GP surgery: Hillcrest Surgery, Bath form online
- visit the surgery and fill out a form
When you register you will also be asked to fill out a medical questionnaire. The form will be sent electronically to our team who will add you to our computer system. Your medical records from your previous doctors surgery will be sent to us. This process may take a few weeks so please ensure you give plenty of time. We do advise people who take regular medications to bring in their repeat prescription into the surgery so we can ensure we have it in good time.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Temporary residents
If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.
To register as a temporary resident you will need to complete a temporary resident form.
You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.
