November 2018

MyScript is BANES’ Social Prescribing Service and is part of the award-winning charity DHI. Through carefully guided conversations we help patients to unpick non-medical issues which may underlie or exacerbate patients’ health conditions. Offering 3 appointments the team act as community navigators to link patients to the community resources they need: this could be housing or debt advice, counselling, activities, opportunities to socialise or manage a long-term condition.

 

Over 91% of our users report an improvement in their wellbeing and all referrals are appropriate; there is almost always something we can offer patients to make some positive lifestyle changes.

We work in GP surgeries, clients' homes and community venues and are open to anyone aged 18+ who is registered with a BANES GP.

October 2018 

Patients in the B&NES area can, from 1st October 2018, also book appointments with a local GP or Nurse in the evening and at the weekend in a B&NES general practice hub, provided by an organisation called BEMS

(BANES Enhanced Medical Services is a GP federation whose members are all the GP Practices in B&NES).

Ask your surgery receptionist for more details about what this service can offer or if you wish to book an appointment with this service.

Annual Health Check Changes

 

If you receive regular health checks and are seen annually for Asthma, COPD, Diabetes, Heart Disease or Stroke we will be changing the date of your annual health check for chronic disease management.

Your review may be sooner or later than you expect as we are moving the review date to the month of your birthday.

 

In future years we would like you to remember that you will be due a review in your birthday month and you should phone the surgery to arrange a suitable appointment and not wait for an invitation.

Regular up to date checks are very important and provide the opportunity to discuss the results and any changes that are required with your care.

 

This change will be introduced over the next six months so please look out for posters, sms text messages and updates on our website.

 

 

 

Appointments

How to book?

Consultations with Doctors or Nurses are made by appointment only.

20% of our appointments are available for pre-booking up to two weeks in advance.

80% of appointments become available on the day. Simply contact us by telephone on the day you need to be seen and we will do our best to accommodate you, if at all possible, with the doctor of your choice. Please try to ring just after 8.00am.

If you do not have a telephone you can call into reception in person from 8.10 am, but you will have to be prepared to return later for your appointment. The surgery Receptionists are available to speak to face to face, from 8.10 am until 6.00 pm.

Please make a separate appointment for each person wishing to be seen.

PLEASE NOTE Fridays are different

On a Friday afternoon we do NOT run routine clinics, we have ONE GP running an urgent care clinic.

Telephone lines open at  1pm when the receptionist will take your details, the Duty Doctor will call you back to assess and book an appropriate appointment.

Routine Appointments

These are scheduled at 10 minute intervals. Some conditions will require less of the Doctor's time and others more. Sometimes it will be necessary to ask you to make a further appointment or if you have several problems to discuss you may be able to request a double appointment at booking.

The patient(s) in front of you may have had a more difficult or complicated problem causing an unavoidable delay, especially if urgent treatment or hospital admission is required. Occasionally, the Doctor may be called out of the surgery to an emergency at a patient's home. If, as a result, your Doctor is behind schedule, we will do our best to keep you informed.

You may see any Doctor in the practice – but remaining with the same one, especially during an episode of illness, will help to improve your care. We encourage where possible patients to see their allocated GP.

Urgent Appointments

We recently changed our appoinmtent system.

Nurse Triage has been replaced with alternative options including:

  • Same day appointment availability with a GP
  • Pre bookable and same day telephone appointments
  • Advice from the nurse will be available on a pre bookable basis as well as same day if medically urgent

These changes will improve availability and access to a whole range of appointments.

Please bear with us during the transition period.

Cancellations

As there is a high demand for appointments, please let us know as soon as possible if you should have to cancel in order that we may pass the appointment onto someone else.
